WSO State Required Forms Upload

Please submit forms ASAP. Deadline is 7:00 am April 10th.

Instructions:

Complete the information in the form below and “UPLOAD” your document packet. See here for 2024 Required Forms.
When you choose “UPLOAD”, you will be taken to a DropBox upload window where you will submit your files and information.
Once you have uploaded your file in the DropBox window, come back to this page and click “SUBMIT”. We realize this is a bit of a duplication, but the second “SUBMIT” allows us to better track the submissions.

NOTES: Please combine all of your forms into one PDF document to upload. For schools who have multiple teams, please upload all of your forms as one PDF. Thank you!
